At Quakecon today Bethesda's Games Director, Todd Howard admitted why the games created by Bethesda never include ladders to climb without a loading screen.

The Director explained that the primary reason why there is no ladders to climb in the game environment is due to the fact that the their engine causes problems for the player character.

    Well, that's interesting....

    I can in fact, imagine both the AI and the character animation for handling ladders would be a bit iffy.

    Bethesda since creating Radiant AI have had a problem getting NPCs to do everything else normally let alone ladders I do not blame them for leaving ladders out.

    Tbh I never even thought about it when playing Fallout, just considered it to be a zone loading issue. Interesting indeed.

    Yup, I hadn't thought of that, and I still don' t really care. It affects the game in no way. Just like how Oblivion doesn't have toilets.

    Still it is an interesting fact, I always wondered why in Oblivion they never climbed ladders or opened doors, but it makes a lot of sense now they have explained why.
